Support Coordination – What is it

The NDIS can be overwhelming and hard to understand.

We know that everybody is unique and requires different supports to achieve their goals and allow their dreams to come true.

It is our priority to ensure that all NDIS participants we support coordinate are the ones who are in control of their own lives.

Our Support Coordinators have worked for many years to gain the knowledge and expertise to be able to successfully support you to navigate your NDIS plan to fulfill your dreams. 

Our Support Coordinators are committed in building positive, personable rapport with NDIS participants to, but not limited to:

• understand and implement their NDIS Plan

• establish and connect to supports most suitable for their needs and goals

• work with them to build capacity and resilience to allow greater choice and control over their life

• access mainstream services and involve these services in planning and coordinating supports

• encourage the setting up of clear, concise service agreements with service providers to maximise value for money

• work through challenges and resolve issues

• prepare for their NDIS plan review and report on the outcomes of their goals
